MONTHLY INJURY REPORTING
To support our Safe for Life Promise, Seqwater requires all contractors (including their subcontractors), consultants undertaking site works (including their subcontractors) and Labour Hire to provide stats relating to works for Seqwater each month. Contractor reports are on by the 20th of each month.
Frequently asked questions
You will have to complete the report if:
- your organisation provided service to Seqwater in the last month.
- those services were carried out at a Seqwater site.
- The data will be aggregated into a single report for the Minister, Seqwater’s Executive Leadership Team and Board to support monitoring of Seqwater’s safety performance.
- All data provided is secured within Seqwater under the Queensland Government Information Security Standard IS18:2018.
Yes. All incidents that occur on Seqwater sites should continue to be reported to your engaging officer and Seqwater’s incident hotline on (07) 3270 4040. If the number of incidents provided in your monthly injury report data don’t match with incidents reported, Seqwater will follow up to obtain additional details of the incident.
Changes to previously reported information are to be entered in the same spreadsheet as the current period information e.g. a previous MTI incident becomes a LTI if time is lost due to the need for surgery/treatment at a later date.
Newly acquired information, for a previous reporting period, is to be entered in the same spreadsheet as your current period information.
Contractors and where applicable consultants and labour hire will be advised of any reporting requirements at the commencement of the engagement.
If not received by the due date, the Seqwater Contract Manager or Superintendent will issue a contract request letter.
